The Caring Hearts Homeless Outreach Program
The Caring Hearts Homeless Outreach Program delivers mobile street outreach to individuals and families experiencing homelessness in East Chicago. Outreach workers seek out individuals in need living on the streets, under viaducts, and other places not meant for human habitation. The team offers basic survival items, food gift cards, blankets, coats, and other necessities. Through consistent contact, the outreach team works to build trust in order to learn about the needs of those served. Outreach workers offer referrals, care coordination, and transportation to services in the community. By means of our agency's Claudia's Closet, a collection site for donated new and gently used clothing items, warm coats, blankets, and shoes, we are able to help those in the homeless community to survive the various dangerous street elements especially during our frigid winter months.

CARING HEARTS HOMELESS PREVENTION AND COMPLIANCE INITIATIVE PROGRAM
Our Caring Hearts Our program is designed to help eradicate homelessness by identifying and addressing the barriers that negatively impact the quality of life for our city’s residents and families who are at risk of eviction leading to their becoming homeless. This unique program brings together community partners with their distinct areas of expertise to focus on implementing outreach, health and supportive service programs to assist at risk participants to remain housed, thereby reducing the number of homeless individuals on our streets and transforming their lives by helping them to attain self-sufficiency and to live a productive and healthy life.
We are currently operating this program in the City of East Chicago, sponsored by The Foundations of East Chicago.
